Clinicians in the Community

The CUAC partners with other senior agencies through a comprehensive range of programs. Their shared goal is to improve the quality of care and quality of life through coordinated services.

At-Home Mental Health Service Program
The CUAC collaborates with Silver Key Senior Services, providers of essential services to home-bound older adults, to identify and treat frail, homebound elderly who are at-risk for physical and cognitive decline. Up to 20 sessions are provided at no cost to the community seniors.

Integrated Mental Health Services with Primary Care
This collaboration with Peak Vista Community Health Clinics integrates behavioral health and primary care at Peak Vista's Senior Health Clinics. In conjunction with their primary care visit, patients can receive on-site services from CUAC clinicians. Depression and anxiety screening is routinely available to all Peak Vista patients. A brief computerized memory screen will be offered in Spring 2008 for early detection of cognitive impairment. Therapy groups for older adults coping with chronic illnesses, including diabetes, are also in development at the Peak Vista Senior Health Clinics.

Mountain View Care Center
This partnership with a local nursing home brings mental health care on-site to residents.
